Education Student Photo Release Form

As part of the marketing for CTC’s Education programming we take photographs and short videos (30-60 seconds) of our programs which may be used for the Children’s Theatre Company’s promotional and development needs.

Below is a consent form asking your permission for us to use recordings or submitted video content of your student for advertisement, marketing, or grant development purposes.
